Our goal is to provide a safe, supportive, and enriching environment for all students and families involved in Prime Mind Learning. To ensure this, we have established the following Code of Conduct, which applies to all students, parents, guardians, tutors and all individuals participating in a Prime Mind Learning event.

  1. Primary Responsibility: I understand that I am the primary instructor of my child. I will remain on site or in class with my child at all times and support the leader and all activities. For older students/teens left on their own with the group, the parent assumes all responsibility for their child(ren), even if they are absent from their child(ren) during an event.
  2. Student Participation: I will ensure that my child will: (a) Participate appropriately, (b) Show respect to the leaders, (c) Show respect to their peers by exhibiting a moral character to those around them. If my child has a difficult time appropriately participating or causes disruption, I will remove my child from the activity. The administrators reserve the right to ask your child to leave the group if there are ongoing disruptive behaviors.
  3. Attendance: Our leaders plan their activities specifically with your child in mind. If you know you will be unable to attend, please let us know in advance. We understand sick days occur unexpectedly, but please contact us as soon as you know you will not be attending.
  4. Punctuality: Please arrive on time and be mindful of the schedule.
  5. Modest Clothing: Please honor others by choosing modest clothing.
  6. Health and Safety: Students who are ill should stay home to prevent the spread of illness. Parents must provide up-to-date emergency contact information.
  7. Communication: All Prime Mind Learning communication will be through email and GroupMe. Please be sure to join your group to stay connected and up to date.
  8. Respect: All members of the homeschool group are expected to treat each other with respect and courtesy at all times.
  9. Safety: Actions that threaten the safety or well-being of others, including bullying, harassment (including sexual harassment), and physical aggression, are strictly prohibited. 
  10. Cooperation: Families are expected to cooperate with leaders and volunteers, follow instructions, and adhere to the group’s policies and procedures.
  11. Open Dialogue: Parents are encouraged to maintain open communication with leadership regarding any concerns or issues. Disagreements should be handled respectfully and constructively.
  12. Care for Property: (a) All members must take care of the facilities and equipment provided by the homeschool group and leave shared spaces tidy. (b) Any damage to property caused by a member, whether intentional or accidental, must be reported immediately to the staff. (c) The responsible party (parents or guardians of the student) will be held financially accountable for the repair or replacement of the damaged property.
  13. Conflict Resolution: I acknowledge that if a conflict arises, I will address it by notifying the founder. I agree that I will not, at any time, directly or indirectly, initiate or pursue any legal action, lawsuit, or other legal proceeding against any individual or organization connected with Prime Mind Learning.
  14. Compliance and Enforcement–Violation of Code of Conduct: If a family or student violates the Code of Conduct, the following steps will be taken: (1) Warning: The family will receive a verbal or written warning outlining the specific violation and expected behavior changes. (2) Meeting: If the behavior continues, a meeting will be held with the family to discuss the issue and create a plan for improvement. (3) Probation: The family may be placed on probation, during which time any further violations will result in immediate action. (4) Dismissal: If the violations continue or are severe in nature, the homeschool group reserves the right to ask the family to leave the program. The decision will be made by the founder and communicated in writing.

Acknowledgment and Agreement: By enrolling in our homeschool group, parents and guardians acknowledge that they have read, understood, and agree to abide by this Code of Conduct. Failure to comply with these standards may result in dismissal from the program.

In consideration of my child(ren) or ward(s) or myself being permitted to participate in any activities of Prime Mind Learning at any location where such activities take place (collectively, the “Activities”), I, for myself, my child(ren) and/or ward(s), my heirs, personal representatives, or assigns, hereby agree not to sue, and release, waive, and discharge Prime Mind Learning, its directors, officers, staff members, volunteers, advisors, property owners, and agents, from any liability and from all claims, including those resulting from the negligence of Prime Mind Learning, its directors, officers, staff members, member families, volunteers, advisors, property owners, and agents, that result in personal injury, accidents or illnesses (including death), and property loss arising from participation in the Activities.

ASSUMPTION OF RISKS: Participation in the Activities carries inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another but include: (1) Minor injuries such as scratches, bruises, and sprains, (2) Major injuries such as eye injuries or loss of sight, joint or back injuries, broken bones or teeth, asthma or heart attacks, and concussions, (3) Catastrophic injuries including paralysis and death.

I have read the previous paragraphs and understand and appreciate these and other risks that are inherent in the Activities, which may include, without limitation, parties, picnics, hiking, swimming, field trips to museums, plays, or parks, recitals, or practices. I assert that my participation and/or the participation of my child(ren) or ward(s) is voluntary and that I knowingly assume and accept all risks. I also understand that I may participate in activities not sponsored or supported by Prime Mind Learning but are private activities of Members, and I assume all risks associated with such private activities (“Non-Prime Mind Learning Activities”).

INDEMNIFICATION AND HOLD HARMLESS: I agree to indemnify and hold Prime Mind Learning, its directors, officers, staff members, volunteers, member families, advisors, property owners, and agents, harmless from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorneys’ fees, brought as a result of my involvement in the Activities or Non-Prime Mind Learning Activities, and/or the involvement of my minor child(ren) or ward(s) in the Activities or Non-Prime Mind Learning Activities, in any location where these activities are conducted. This indemnification extends to all claims which may hereafter develop or accrue to them on account of injury, loss, or damage, which may be suffered by said minor or to any property, because of any matter, thing, or condition, negligence, or default whatsoever, and to reimburse them for any such expenses incurred.

MEDICAL CONSENT: The Parent/Guardian grants permission for the Organization to seek medical treatment for the Student in the event of an emergency and agrees to assume responsibility for all costs associated with such treatment.

SEVERABILITY: The undersigned further expressly agrees that the foregoing waiver, assumption of risks, indemnification, and hold harmless agreement is intended to be as broad and inclusive as is permitted by the law, and that if any portion thereof is held invalid, it is agreed that the balance shall continue in full legal force and effect. This Agreement shall be governed by and construed in accordance with the laws of the state of Utah, without regard to its conflict of law provisions.

ENTIRE AGREEMENT: This Agreement constitutes the entire agreement between the parties and supersedes all prior agreements, representations, and understandings, whether written or oral, relating to its subject matter.

ACKNOWLEDGMENT OF UNDERSTANDING: I have read this Waiver of Liability, Assumption of Risks, and Indemnity Agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

PHOTO RELEASE: I hereby grant Prime Mind Learning (“the Group”) permission to use my/my child’s likeness in photographs, video recordings, and other media in any and all of its publications, including online and social media platforms, without payment or any other consideration. I understand and agree that: (1) The Group may use the photos/videos in printed materials, websites, social media, newsletters, and other promotional materials. (2) I waive any right to royalties or other compensation related to the use of these materials. (3) All photos, videos, and recordings will become the property of the Group. (4) I release and hold harmless the Group, its organizers, volunteers, and affiliates from any claims, demands, or liabilities related to the use of these images or videos. (5) I understand that I may revoke this permission at any time by submitting a written request to the Group, but any previous materials already published will not be affected.
